Who this covers
Cheryl L. Andrews & Associates provides Clinical Function and Accommodation Consultation, Function-Focused Psychotherapy, and Clinical Supervision from offices in Charlotte, North Carolina and Atlanta, Georgia, and by secure telehealth in North Carolina, South Carolina, Georgia, and Virginia.
This policy describes how the practice handles two different kinds of information: protected health information created or received in the course of providing care, and website information collected when you visit cheryllandrews.com.

Protected health information
Health information created or received in the course of consultation, psychotherapy, or supervision is confidential and is protected under the Health Insurance Portability and Accountability Act (HIPAA) and applicable state law. It is used and disclosed only as follows:
- Treatment — to provide, coordinate, or manage your care, including consultation with other providers involved in your care.
- Payment — to bill and collect payment for services, including submitting claims to a health plan or employee assistance program where applicable.
- Health care operations — for quality review, clinical supervision, training, and administration of the practice.
- With your written authorization — for any other purpose, including communication with an employer, school, attorney, insurer, or other third party. Authorization is specific in scope and may be revoked in writing at any time.
- Where required or permitted by law — including circumstances involving suspected abuse or neglect, a serious and imminent threat to health or safety, court orders, and mandatory reporting obligations under the law of the state in which services are provided.
Third-party communication
The practice does not contact an employer, school, human resources department, provider, insurer, or attorney about you without your written authorization and a clearly defined scope. This applies to consultation and psychotherapy alike.
Where a referral is received from a provider, employer, or human resources team, receipt of the referral does not by itself authorize any communication back to the referring party. That requires your separate written authorization.
Your rights regarding your health information
- Request access to, and a copy of, your records
- Request an amendment to your records
- Request an accounting of certain disclosures
- Request restrictions on certain uses and disclosures
- Request confidential communication by a particular method or at a particular address
- Receive a paper copy of the Notice of Privacy Practices
- Be notified in the event of a breach affecting your unsecured protected health information

This website
cheryllandrews.com is an informational website. It does not collect health information, and it has no contact form, patient portal, or account login. Nothing you type into this site is transmitted to the practice, because there is nothing on this site to type into.
The site is served as static pages. It sets no advertising or tracking cookies of its own. Standard web server logs, kept by the hosting provider, may record technical information such as IP address, browser type, and pages requested, for security and reliability purposes.
Typefaces are loaded from Google Fonts, which may receive your IP address as part of delivering the font files.
Links to other services
Several buttons on this site open scheduling, referral, and records services operated by other companies:
- The scheduling and patient portal used for virtual psychotherapy appointments and intake paperwork
- The consultation and supervision booking pages used to request appointment times
- The referral form used by providers, employers, and human resources teams
- The Vocational Standard, a separate firm handling forensic vocational, legal, and claims matters
Once you leave this website, the information you provide is governed by the privacy policy of that service, not by this one. Information you submit through the scheduling portal and intake packet is treated as protected health information by the practice once received.
Email, text, and fax
Email and text messages are not fully secure. Please do not send detailed health information by email or text. Routine scheduling and administrative messages may be exchanged this way if you choose; anything clinical is better handled by phone or through the secure portal.
Changes to this policy
This policy may be updated. The effective date at the top of the page reflects the most recent revision, and the current version is always posted here.
